The first is the builder-champion mismatch: the executive who signs a contract is rarely the person who does the work, so programs handed off to an already-busy director tend to starve. His rule is that if the sponsor can't name the three people who will build the first three workflows, don't sign. The second is the enterprise-wide kickoff. He contrasts a construction firm that rolled out an HR chatbot to 2,000 people on day one (adoption collapsed within a week) against a mid-market bank that gave access to just 14 people in one department, targeting tasks everyone already hated. The third mistake is the 50-use-case master plan. Fourth is the duplicate agent problem, illustrated by an engineering firm that built the same invoice-processing agent four times. His fix is a central, searchable registry of agents, set up before an organization has ten of them. Fifth, the island problem, covers platforms that connect to nothing teams actually use; every custom integration adds weeks, so integration depth should be evaluated before committing. Sixth is the fear factor, where anxiety over data incidents keeps people working manually. Organizations that break through choose platforms with governance built in: role-based permissions, full audit trails, guardrails, and human approval steps that replace fear with trust.
